Social Work
The Center provides mental health social work services at Center Health Care. Our licensed Social Workers provide psychosocial Article 16 and Article 28 services. Services include interdisciplinary care that may involvepsychology, speech therapy, occupational therapy, physical therapy, rehabilitation counseling, and social work.
Article 16 Clinic Services
Article 16 clinics are certified by the New York State Office for People With Developmental Disabilities (OPWDD) and specialize in providing services to individuals with intellectual and developmental disabilities (I/DD). These clinics offer interdisciplinary care that may include psychology, speech therapy, occupational therapy, physical therapy, rehabilitation counseling, and social work.
These services focus on improving quality of life, increasing independence, promoting community inclusion, and helping individuals achieve their highest level of functioning.
Services include:
- Conducting psychosocial assessments.
- Providing individual, family, and group counseling.
- Assisting individuals and families in understanding diagnoses and treatment plans.
- Connecting clients with community resources, benefits, housing, educational supports, and respite services.
- Advocating for clients during transitions between school, community, and adult services.
- Collaborating with psychologists, physicians, therapists, and care managers to develop person-centered treatment plans.
- Supporting crisis intervention and behavioral health needs when appropriate.
Article 28 Clinic Services
Article 28 clinics are licensed under the New York State Public Health Law and include hospitals, diagnostic and treatment centers, and outpatient medical clinics. These clinics provide comprehensive medical and behavioral healthcare services.
Services include:
- Screen patients for psychosocial needs.
- Complete comprehensive psychosocial assessments.
- Provide crisis intervention and supportive counseling.
- Help patients cope with illness, injury, disability, or hospitalization.
- Coordinate discharge planning and continuity of care.
- Connect patients with financial assistance, insurance resources, transportation, home care, and community services.
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to improve patient outcomes.
